Enrollment for SY 2019 – 2020

We are excited to begin the enrollment process for the 2019-2020 School Year!

Enrolling is as easy as 1, 2, 3! Please review the guidance below to complete your enrollment for the Howard University Middle School of Mathematics and Science – (MS)2.

 

STEP 1 – Electronic Enrollment Form

Families who were matched with our school will receive an email welcoming you to the (MS)2 family. Please follow the directions in the email to complete the electronic enrollment form by Wednesday, May 1. STEP 2 – Supporting Documents

Find below the list of documents and paper forms to be submitted to the school:

  1. My School DC Seat Acceptance Form
  2. DC Residency Verification Form & Supporting Documents (All acceptable supporting documents are listed on the reverse side of the DC Residency Verification Form.)
  3. IEP (Individualized Education Program) or 504 Plan (if applicable)
  4. Current Health Forms (not expired)

Please find below the official health related forms. The forms have to be printed and filled out by the student’s physician. The forms will also need the signature of the parent or legal guardian as per the instructions on each form. Every student should have an updated health (which includes immunization records) and dental exam to enroll.

  1. District of Columbia Universal Health Certificate. This form has to be completed by the student’s physician – Click Here
  2. District of Columbia Oral Health Assessment Form. This form has to be completed by the student’s dentist – Click Here

 

Failure to submit any of the required documents included in this checklist by the listed due date will result in your child’s place at (MS)2 being forfeited and offered to the next student on our waiting list. The submission of any fraudulent information or the withholding of information can result in the revocation of this offer of enrollment and expulsion.

 

STEP 3 – New Family Orientation

Family Interview

Once you have submitted the required paper forms, you will have the opportunity to schedule a family interview to meet with members of our school community. Family Interviews are a mandatory part of our student-family orientation process. This interview gives us a chance to learn more about each new student to ensure that they are supported and successful in our program. It also allows us to share important parts of our program with you, and answer any questions you may have as you prepare to transition your child to a new school.

 

 

Summer Bridge (July 15 – July 26)

Beginning on Monday, July 15, we will welcome all of our new students to (MS)2 for our two-week orientation program, designed to introduce them to their new school! This is an opportunity for students to meet their classmates and teachers, get to know the school rules, learn about the school’s technology and other aspects of our school culture to help ensure that they are prepared for success at (MS)2.
