Enrollment for SY 2021 - 2022
We are excited to begin the enrollment process for the 2021-2022 School Year!
Please review the guidance below and follow this two-step process to complete your child’s enrollment for the Howard University Middle School of Mathematics and Science – (MS)2.
STEP 1 - Complete Electronic Enrollment Form
Families who are matched with our school will receive an email welcoming you to the (MS)2 family. Please follow the directions in the email to complete the electronic enrollment form by Monday, May 3.
PLEASE NOTE: You MUST upload the parent/guardian ID and student birth certificate to verify your identity and confirm that you are the legal parent/guardian of the student and thus authorized to enroll him/her.
By accepting a space at a new school, you acknowledge that you are giving up your child’s seat at your current school, forgoing feeder rights to schools related to your child’s current school, and your child will be removed from the waitlists of schools ranked below our school on their My School DC application.

STEP 2 – Submit Supporting Documents
Once you have submitted the New Student Registration Form, you will receive an email via DocuSign inviting you to sign and submit all additional required documents to complete your child’s enrollment.
Required Documents Checklist (All documents must be submitted electronically OR in-person by the deadline.)
- DC Residency Verification Form & supporting documents to prove DC residency. (All acceptable supporting documents are listed on page 2 of the DC Residency Verification Form.)
- Current Health Forms (unexpired)
The attached forms have to be filled out by the student’s physician. The forms will also need the signature of the parent or legal guardian as per the instructions on each form. Every student should have an updated health (which includes immunization records) and dental exam to enroll.*
- District of Columbia Universal Health Certificate. This form has to be completed by the student’s physician – Click Here
- District of Columbia Oral Health Assessment Form. This form has to be completed by the student’s dentist – Click Here
- Photo Waiver/Universal Permission Form
- IEP (Individualized Education Program) or 504 Plan (if applicable)
*In the event that your child does not have updated medical and dental forms, parents must present appointment cards to confirm that your child is scheduled to complete their physical and dental exams in the near future. Forms must be shared with the school immediately following the appointment.
Failure to submit any of the required documents included in this checklist by the listed due date will result in your child’s place at (MS)2 being forfeited and offered to the next student on our waiting list. The submission of any fraudulent information or the withholding of information can result in the revocation of this offer of enrollment and expulsion.
